What Trulicity® Is and How It Works
Trulicity® (dulaglutide) is a once-weekly GLP-1 receptor agonist for adults with type 2 diabetes. It improves blood glucose control and, in adults with established cardiovascular disease, has been shown to reduce the risk of major adverse cardiovascular events. Trulicity activates the GLP-1 pathway to increase glucose-dependent insulin secretion, slow gastric emptying, and reduce glucagon. Many patients also experience modest weight loss over time. It is used alongside diet and exercise and may be combined with other diabetes medicines when appropriate.
Trulicity is supplied as prefilled, single-dose autoinjector pens. The device hides the needle and delivers medication in a few quick steps, designed to make weekly injections simple and discreet.
Dosage and Usage
Trulicity is available in several strengths: 0.75 mg/0.5 mL, 1.5 mg/0.5 mL, 3 mg/0.5 mL, and 4.5 mg/0.5 mL. It is injected subcutaneously once weekly, on the same day each week, with or without food.
- Initiation: Start at 0.75 mg once weekly.
- Titration: Increase to 1.5 mg after at least 4 weeks if additional control is needed.
- Further increases: Step up to 3 mg after at least 4 additional weeks, then to a maximum of 4.5 mg once weekly if required.
- Missed dose: If there are at least 3 days (≥72 hours) until the next scheduled dose, administer as soon as possible. If fewer than 3 days remain, skip the missed dose and resume the regular schedule.
- Injection sites: Abdomen, thigh, or upper arm. Rotate sites each week to reduce irritation.
- How to inject: Wash hands, inspect the pen and solution, and clean the injection site. Remove the cap, place the base flat against the skin, unlock, and press the button. Hold until the first click, wait for the second click, then keep the pen in place for a few seconds before removing.
- Timing: Take on the same weekday each week. If needed, the injection day can be changed, provided there are at least 3 days between doses.
Storage and Travel
- Refrigerate at 2–8 °C (36–46 °F). Do not freeze. Protect from light.
- Pens may be stored at room temperature (up to 30 °C / 86 °F) for up to 14 days.
- Do not use if frozen, dropped with damage, or if the solution is cloudy, discoloured, or contains particles.
- Keep pens in the original carton during storage and transport.
- For travel: use an insulated pouch with cold packs (avoid direct contact with ice) and carry in hand luggage. Have a backup plan for refrigeration at your destination.
Benefits
Trulicity helps lower A1C and fasting glucose by enhancing insulin secretion and slowing gastric emptying. Clinical studies show meaningful improvements in blood sugar and modest weight loss in many patients. In those with type 2 diabetes and established cardiovascular disease, Trulicity also reduced the risk of major adverse cardiovascular events. Weekly dosing supports adherence, and the autoinjector device offers a simple, low-step injection experience.
Side Effects and Safety
- Common: Nausea, vomiting, diarrhoea, abdominal pain, decreased appetite, constipation, indigestion.
- Other: Fatigue, injection-site reactions, mild dehydration from gastrointestinal losses.
- Serious or rare: Pancreatitis, gallbladder disease, acute kidney injury (often linked to dehydration), severe gastrointestinal disease, or hypersensitivity.
Trulicity carries a boxed warning for risk of medullary thyroid carcinoma (MTC). It should not be used in patients with a personal or family history of MTC, or with multiple endocrine neoplasia syndrome type 2 (MEN 2). It is not for type 1 diabetes or diabetic ketoacidosis. Discuss pregnancy and breastfeeding with your healthcare provider before starting treatment.
Onset Time
Some patients notice lower fasting glucose within the first 1–2 weeks. A1C reduction typically builds over 4–12 weeks as the dose stabilises. Weight changes may begin within the first month and continue over several months. The full effect on A1C is usually assessed after at least three months on a stable dose.
Compare With Alternatives
Rybelsus® (oral semaglutide): a daily tablet that may be suitable for patients who prefer oral medication, though strict dosing rules apply.
Mounjaro® (tirzepatide): a once-weekly dual GIP/GLP-1 receptor agonist that has shown robust A1C and weight-loss effects in trials.
Wegovy® (semaglutide 2.4 mg): indicated specifically for chronic weight management in adults with obesity or overweight plus related conditions.
Combination Therapy
- Metformin: Trulicity is often added to improve A1C when metformin alone is insufficient.
- SGLT2 inhibitors: Work via complementary mechanisms and may improve glycaemic and cardiovascular outcomes.
- Basal insulin: Can be used together, though insulin dose adjustments may be needed to reduce hypoglycaemia.
- Sulfonylureas: Monitor closely for low blood sugar; dose reductions may be needed.
- Other cardiometabolic agents: Statins, ACE inhibitors, and related medications are often co-prescribed for overall risk reduction.
Patient Suitability
Trulicity may be appropriate for adults with type 2 diabetes who require better blood sugar control despite diet and exercise. It is especially beneficial for those with cardiovascular disease who may gain additional heart protection. It is not suitable for patients with MTC, MEN 2, or a history of severe hypersensitivity to dulaglutide. Caution is advised for patients with pancreatitis history, gallbladder disease, or severe gastrointestinal disorders.

What is Trulicity used for and how often is it taken?
Trulicity® (dulaglutide) is a GLP-1 receptor agonist for adults with type 2 diabetes. It helps lower blood glucose and may support modest weight loss. Trulicity is injected once weekly, on the same day each week, with or without food. It may be used alone or alongside other diabetes medicines as directed by your clinician.
What doses does Trulicity come in and what is the maximum dosage?
Trulicity is available in 0.75 mg, 1.5 mg, 3 mg, and 4.5 mg single-dose pens. Many patients start at 0.75 mg once weekly and may increase every 4 weeks depending on blood glucose control and tolerability. The maximum labelled dose is 4.5 mg once weekly. Any dose adjustments should be guided by your healthcare provider.
How do you use the Trulicity pen?
Before use, wash your hands and check the pen and solution. Choose an injection site on the abdomen, thigh, or upper arm. Remove the cap, place the base flat against the skin, unlock, and press the button. Keep it pressed until the second click is heard, then wait a few seconds before removing. Rotate injection sites each week to reduce irritation.
What are common Trulicity side effects?
The most common side effects include nausea, vomiting, diarrhoea, abdominal pain, decreased appetite, and constipation. Injection-site reactions and fatigue may also occur.
Serious but less common risks include pancreatitis, gallbladder disease, and allergic reactions. Trulicity carries a boxed warning about thyroid C-cell tumours and should not be used by patients with a personal or family history of medullary thyroid carcinoma (MTC) or in those with multiple endocrine neoplasia syndrome type 2 (MEN 2).
When combined with insulin or sulfonylureas, the risk of hypoglycaemia may increase. How long does Trulicity take to work?
Some patients may notice lower fasting glucose within 1–2 weeks. A1C reductions typically build over 4–12 weeks as the dose is adjusted and stabilised. Weight changes may begin within the first month and continue over several months. Clinicians often reassess control after at least 3 months on a stable weekly dose.
How should Trulicity be stored, including room temperature guidance?
Store pens in the refrigerator at 2–8 °C (36–46 °F) and protect from light. Do not freeze. If needed, pens can be kept at room temperature (up to 30 °C / 86 °F) for up to 14 days. Keep them in the original carton during storage and travel. Discard pens that were frozen, damaged, or show cloudy or discoloured solution.
How much does Trulicity cost?
