What Mounjaro® Is and How It Works
The Mounjaro® KwikPen Pre-Filled Pen contains tirzepatide, a once-weekly injection for adults with type 2 diabetes. It activates both GIP and GLP-1 receptors to help lower blood glucose and reduce appetite. The device is a single-use, prefilled pen designed for subcutaneous injection.
In clinical studies (SURPASS programme), tirzepatide showed robust A1C reductions and meaningful weight loss when added to diet and exercise. It is not a substitute for insulin in type 1 diabetes and is not approved for diabetic ketoacidosis. The KwikPen delivers set doses; each pen is single-use and contains one weekly dose.
Dosage and Usage
- Initiation: 2.5 mg once weekly for 4 weeks to start therapy.
- Titration: Increase to 5 mg once weekly. If needed, raise by 2.5 mg steps (7.5 mg, 10 mg, 12.5 mg, 15 mg) at 4-week intervals as tolerated.
- Administration: Subcutaneous injection in abdomen, thigh, or upper arm. Rotate injection sites with each dose.
- Timing: Take on the same day each week, any time of day, with or without meals.
- Missed dose: If within 4 days (96 hours) of the missed dose, take it as soon as possible and resume the regular weekly schedule. If more than 4 days have passed, skip the dose and take the next dose on the usual day.
- Spacing: Keep at least 3 days (72 hours) between doses when adjusting schedules.
- Device steps (general): Inspect the pen; do not use if damaged or cloudy/discoloured. Wash hands. Choose an injection site and clean with alcohol. Unlock and place flat against the skin. Press and hold as directed until the injection is complete per the pen’s instructions for use. Dispose of the used pen in a sharps container.
- Refrigerated storage: Keep pens in a refrigerator at 2–8 °C (36–46 °F). Do not freeze. Protect from light by storing in the carton.
- Room temperature: Pens may be kept at room temperature up to 30 °C (86 °F) for up to 21 days. Do not return to the refrigerator after room temperature storage.
- Handling: Do not shake. Do not use if the pen has been frozen, dropped on a hard surface, or appears damaged.
- Travel: Use an insulated cool bag with ice packs; avoid direct contact with ice. Keep the pen with you, not in checked luggage. Do not leave in a hot car. Carry extra supplies and a sharps container or travel-safe alternative.
- Disposal: Place used pens in an FDA-cleared sharps container or a heavy-duty household container with a tight-fitting lid.
Benefits
Tirzepatide lowers A1C significantly, often around 2 percentage points at higher maintenance doses in trials. Many patients also see reduced fasting and post-meal glucose, fewer glycaemic excursions, and weight loss that builds over several months. Once-weekly dosing supports adherence, and the single-use pen simplifies injections.
Convenience extends to travel and storage flexibility, with a 21-day room-temperature window when needed.
Side Effects and Safety
- Common effects: nausea, diarrhoea, constipation, vomiting, abdominal pain, indigestion, decreased appetite, and belching.
- Other reported: fatigue, dizziness, injection-site reactions, and mild dehydration symptoms.
- GI tolerance: effects often lessen over time and after dose adjustments.
- Low blood sugar: risk increases when used with insulin or sulfonylureas; dose reductions of these agents may be considered.
Serious risks are uncommon but can include acute pancreatitis, gallbladder problems, severe GI symptoms, kidney issues from dehydration, and allergic reactions. Tirzepatide carries a boxed warning about the potential risk of thyroid C-cell tumours. Do not use in patients with a personal or family history of medullary thyroid carcinoma (MTC) or in multiple endocrine neoplasia syndrome type 2 (MEN 2). Seek urgent care for symptoms such as severe abdominal pain, persistent vomiting, or signs of an allergic reaction.
Onset Time
Blood glucose improvements can begin within the first few weeks as weekly dosing starts. A1C reductions become clearer by 8–12 weeks and continue as the dose increases. Weight changes may start in the first month and often build through 3–6 months. Most patients reach a stable maintenance dose by week 12–24, after which effects tend to plateau.
Compare With Alternatives
Trulicity® (dulaglutide): another once-weekly GLP-1 receptor agonist for type 2 diabetes. It offers A1C reduction and weight benefits, though head-to-head data suggest tirzepatide generally provides greater glycaemic and weight effects at higher doses.
Rybelsus® (oral semaglutide): a GLP-1 therapy in tablet form taken once daily. It may suit patients preferring an oral option, though strict administration timing is required for absorption.
Wegovy® (semaglutide 2.4 mg): indicated for chronic weight management. It is a GLP-1 agonist given once weekly and may be considered when weight reduction is a primary goal.
Combination Therapy
- Metformin: commonly used together as foundational therapy.
- SGLT2 inhibitors: complementary mechanisms for additional A1C and weight benefits.
- Basal insulin: combination is possible; lowering insulin doses may reduce hypoglycaemia risk.
- Sulfonylureas: consider dose reductions to minimise hypoglycaemia.
- Statins, antihypertensives, and other chronic therapies: no direct interaction expected, but monitor overall tolerability.
Patient Suitability
Adults with type 2 diabetes who need additional glycaemic control alongside diet and exercise may be candidates. It is not for type 1 diabetes or diabetic ketoacidosis. Avoid use in those with a personal or family history of MTC or with MEN 2. Use caution in patients with pancreatitis history, severe GI disease, or significant dehydration risk.
Pregnancy and breastfeeding require individualised clinical guidance. Renal function and hydration should be considered, especially if significant GI symptoms occur. Dose escalation should follow the labelled schedule to support tolerance.

Do I need a prescription for Mounjaro?
Yes. Mounjaro® (tirzepatide) is a prescription medication and must be prescribed by a licensed healthcare professional.
How is Mounjaro KwikPen dosed and increased over time?
Most patients start at 2.5 mg once weekly for 4 weeks, then increase to 5 mg. If further control is needed, the dose may be increased in 2.5 mg steps (7.5 mg, 10 mg, 12.5 mg, up to 15 mg) at 4-week intervals as tolerated. Injections are given on the same day each week, with or without food. Rotate between injection sites (abdomen, thigh, or upper arm) to reduce irritation.
What if I miss a weekly Mounjaro dose?
If it has been 4 days (96 hours) or less since the missed dose, take it as soon as possible and continue your regular schedule. If more than 4 days have passed, skip the missed dose and take your next dose on the usual day. Always keep at least 3 days (72 hours) between injections.
How should I store and travel with the KwikPen?
Unopened pens should be refrigerated at 2–8 °C (36–46 °F) and protected from light. Do not freeze or shake. If needed, pens may be kept at room temperature (up to 30 °C / 86 °F) for up to 21 days.
For travel, use an insulated bag with ice packs (avoid direct contact with ice), carry pens in your hand luggage, and avoid leaving them in hot cars. Dispose of used pens safely in a sharps container. What are common side effects of tirzepatide?
The most common side effects are nausea, diarrhoea, constipation, vomiting, abdominal pain, decreased appetite, indigestion, and mild injection-site reactions. These often improve over time.
Serious but less common risks include pancreatitis, gallbladder problems, kidney issues linked to dehydration, and allergic reactions. Seek immediate medical help if you experience severe abdominal pain, persistent vomiting, or signs of an allergic reaction. How soon might benefits appear?
Some patients notice improvements in blood sugar within the first few weeks. A1C reductions usually become clearer by 8–12 weeks as the dose increases. Weight changes often begin within the first month and may continue over 3–6 months. Most patients reach a stable maintenance dose by 12–24 weeks, when benefits typically level off.
What affects the cost of Mounjaro?
The price depends on factors such as dose strength, quantity dispensed, pharmacy pricing, and insurance coverage. Multi-month prescriptions may sometimes reduce overall cost. Ask your healthcare provider or dispensing pharmacy for the most up-to-date information.
